How to access our service

You can self refer, or be referred, into our bereavement service. The easiest way to do this is using our online referrals portal.

We're able to accept referrals based upon the following criteria:

  • Bereavement support offered to Next of Kin or main carer of Farleigh patients
  • Other family members/carers/friends of Farleigh patients on request, via IPU, community teams or direct to the service
  • Any adult 18 or over who has experienced a significant bereavement and it is impacting upon their ability to manage their life
  • Must be registered with a GP in the mid Essex area covered by Farleigh Hospice*
  • Referrals generally not accepted until at least four weeks after a bereavement.

If you're unable or do not wish to complete an online referral, please download and complete the paper referral and self-assessment forms below and return to circle@farleighhospice.org or Farleigh Hospice, North Court Road, Chelmsford, Essex, CM1 7FH.
