Welcome to the Farleigh Hospice e-referral service. Please follow the instructions below to allow us to provide you with the correct referral questionnaire.

Who is completing the referral?

Healthcare Professional Self-Referral Carers/Family Members - referring someone you care for

We welcome referrals from:

  • Anyone over 18 years old and living with or affected by a life-limiting illness, including lung disease, neurological conditions, heart failure, and cancer, living in mid Essex and/or registered with a mid Essex GP
  • Anyone 18+ and registered with a mid-Essex GP seeking bereavement support or information (Circle)
  • Children and Young people aged 4 to 18 living in mid Essex and registered with a mid Essex GP who need pre-bereavement/bereavement support (Yo-Yo Project)
  • Anyone who has given their consent to a referral to Farleigh Hospice

Access to Farleigh Hospice's services may be appropriate at any stage of your illness. Living with a life-limiting illness can affect every aspect of your life and those around you. Farleigh Hospice understands this and offers a range of services to support you and those close to you.

Additionally, adults can access our bereavement service 4 weeks following a bereavement, and children 6 weeks following a bereavement. Children can access our pre-bereavement support at any time.

If your enquiry is regarding adult bereavement services please call 01245 457308 (Circle), or 01245 457416 (Yo-Yo Project) if your enquiry is regarding our bereavement/pre-bereavement service for children and young people, during office hours 9am-5pm.
