Hospice Navigator

As a Hospice Navigator, the post holders will support patients and carers to navigate the clinical services within the Hospice.

By being up to date with all aspects of Farleigh Clinical support themselves, the navigator will be able to direct patients, based on their initial presenting problem, to the most appropriate team.

This role will be focussed on providing general information and support about the hospice and Hospice services, to enable people to navigate the health and social care system and make choices that are best for them. Strong links with the Hospice Rapid Response teams, Community CNS’s and Hospice at Home all ensure the patient experience is positive.

The successful candidate will have highly developed communications and administrative skills, knowledge of Systmone would be advantageous. Knowledge of the hospice services will be developed in induction and support given. Excellent time management and an ability to prioritise are essential to the post.

We are seeking to recruit two experienced, highly motivated individuals to join our Senior Clinical Leadership Team, to support development of services and sustain high quality care.

The post of Head of In-Patient Unit is a senior post within the Clinical Directorate at Farleigh Hospice, Chelmsford. The overall aim of this post will be to lead, manage and co-ordinate the In-Patient Unit.

The role of the postholder will be not only to oversee the services that help ensure that both patients and carers receive a co-ordinated care package in line with the ongoing hospice vision, mission and strategy, but to also manage and lead their whole team to work to the organisational values within their area .

Registered Nurse - Hospice at Home

We are looking for Registered Nurses to join the Hospice, working within our Hospice at Home team. The Hospice at Home service takes the specialist skills of Farleigh Hospice into an individual’s home, enabling people to have more choice over their place of care.

You will be responsible for providing high standards of specialist palliative care in the community setting, where patients have complex symptom control issues, and provide emotional and psychological support to patients and their families. You will deliver hands on nursing care in the short term to patients within their home. You will need to work autonomously within the limits of existing knowledge and skills, demonstrating sound judgment and be able to communicate in a sensitive manner along with an ability to work effectively within a multidisciplinary team and collaborate with our colleagues in health, social and voluntary sectors.

We would like to hear from nurses who have experience in a hospital/community setting, specialities include but not limited to; oncology, palliative care, general medical and surgical, accident and emergency, critical care and community/district nursing

You will be required to demonstrate your abilities in the following areas:

  • First level registered General Nurse
  • Palliative care experience
  • Up to date and relevant qualifications / training
  • Be organised and have strong decision-making skills
  • Ability to work autonomously, with minimal supervision
  • Capability to work remotely and flexibly

You will be required to work a variable shift pattern between the hours of 08.00 and 20.00 over 7 days. You will receive an unsocial hour’s enhancement for weekend and bank holiday working. All candidates must have a full clean driving license and have access to a car.
