Team Lead Hospice Navigator

Hours per week: 37.5

Type of Contract: Permanent

                             - shifts to cover 08.00 – 20.00 Monday to Friday and 08.00 – 16.00 over a weekend

Salary: £19,941 - £22,467 per annum

Interview Date: 5 February 2019 

As the Team Lead Hospice Navigator, the post holder will manage the day to day running of the service and support patients and carers to navigate the clinical services within the Hospice.

By being up to date with all aspects of Farleigh Clinical support themselves, the navigator will be able to direct patients based on their initial presenting problem to the most appropriate team. This role will be focussed on providing general information and support about the hospice and Hospice services, to enable people to navigate the health and social care system and make choices that are best for them.

Strong links with the Hospice Rapid Response teams, Community CNS’s and Hospice at Home all ensure the patient experience is positive.

The successful candidate will have highly developed communications and administrative skills, knowledge of Systmone would be advantageous. Previous experience in a team lead role would be an advantage.

Knowledge of the hospice services will be developed in induction and support given. Excellent time management and an ability to prioritise are essential to the post.     .
