Registered Nurse – In-Reach

Hours per week: 37.5

Type of Contract: Permanent

Salary range: £25,099 to £29,527 per annum

Farleigh Hospice exists to meet the needs of local people affected by life-limiting illnesses and to support those who have been bereaved. By giving people choice and involvement in the care they receive we make a real difference when and where it matters most. We are moving to improve access to services for all meeting a wider range of needs and looking to recruit to posts key to this development.

We are looking for a caring, highly motivated individual to join our community team. The successful applicant will help and support the discharge of rapidly deteriorating patients who want to be discharged to the community as their preferred place of care. 

You will need to be able to build relationships with a variety of stakeholders, including the acute Trust clinical team, primary care, social care and other key care providers as well as the patients and their family members.
