An exciting opportunity has arisen for a part time Motor Neurone Disease Co-ordinator to join the Day & Therapy Service team at Farleigh Hospice. The key purpose of this role is to facilitate co-ordinated, timely and responsive care to meet the individualised holistic needs of people with Motor Neurone Disease (MND) who live in Mid Essex.

For this Co-ordinator role we need someone who is a self-starter with a high degree of accuracy along with proven organisational and self-management abilities. Exceptional collaboration skills are required with the ability to communicate clearly and effectively is essential.

You will have knowledge and understanding of relevant health and social care services and experience of co-ordinating, organising and networking across various sectors. You will have an understanding of the effects of terminal illness and its impact; together with disability issues and the needs of carers, community care provision and legislation.

On a practical level you will need to be proficient in IT skills including Microsoft word, excel and use of data bases and have effective telephone skills.
