Please Note: This role is exempt from the provisions of the Rehabilitation of Offenders Act 1974 (Exemptions) Order 1975. You must therefore disclose any criminal convictions, however long ago these occurred, if the application is to proceed.

The appointment to some volunteer roles will be made subject to an enhanced Disclosure and Barring (DBS) check.

Section 6 - Declaration

I declare that the information which I have given in this application form is true and complete. I understand that if it is subsequently discovered that any statement is false or misleading, this may result in my being asked to discontinue volunteering with Farleigh Hospice. I also give consent for Farleigh Hospice to approach my nominated referees either before or after interview, as per my specification in section 3 of this application form.

Farleigh Hospice will collect, securely store and use information provided by you on this application form for recruitment and selection purposes. You have the right to ask to see this data if you wish. Please contact [email protected] for more details.

As a registered charity, we rely heavily on the ability to communicate with our supporters to help raise the funds it costs to provide our services free of charge each day. The purpose of these communications are to keep you up to date with news of any future developments at the hospice, to tell you more about how your donations and support are helping those we care for, to highlight other ways in which you can choose to support our work, and to raise awareness of our charitable status and high-dependency on voluntary donations to keep our vital local services free of charge for all of the individuals we care for.

We will only send you information if you have consented for us to contact you in this way. If you do not consent for us to contact you, then we are legally unable to invite you to any future events or send you our newsletters. We will endeavour to provide this statement on all marketing communications. Please be aware that there will be a period of up to 6 weeks to update your communication preferences once a request has been made, and as a result, in this time, you may still receive communications from us whilst your request is being processed.

The data we gather and hold is currently managed in accordance with the General Data Protection Regulation (GDPR) and Data Protection Act 2018. The personal information you give us is used exclusively by the Farleigh Hospice Group to keep you up to date with the difference your support is making and to send you occasional information which we think will be of interest. We will not share, sell or swap your personal information with any third party organisation for their own marketing or fundraising purposes.
