Farleigh Hospice are delighted to announce that thanks to additional funding provided by the local NHS, they will be able to enhance the essential services they provide to the local community.

Farleigh Hospice’s Clinical Advice Line and Hospice at Home services will not only provide care to patients in their home environments, but will also provide support to GPs, Hospitals, Ambulance staff, Care Homes, Social Workers, and other healthcare professionals who are involved with a person affected by life-limiting illness.

This joint-working initiative seeks to improve end of life care across mid Essex, even if the patients are not known to the hospice.

Clinical Advice Line

Farleigh Hospice’s Clinical Advice Line provides access to a Hospice Nurse who is able to give advice on hospice care and support, symptoms, nursing care, and other needs relating to life-limiting illness.

The advice line is available on 01245 455478 between 8am and 8pm, 7 days a week.

This service is available to people who have a life-limiting illness, their family and carers, and any relevant healthcare professionals.

Hospice at Home

Farleigh Hospice’s Hospice at Home ([email protected]) team provide practical help and emotional support for a patient who has reached the end stage of their life and whose preference is to spend their last few weeks in their usual place of residence.

The service works collaboratively with other services and departments (for example GPs, Community Nursing Team, Hospital staff, Social Services) to provide care and support to enable patients to remain in their preferred place of care.

They can provide practical support and hands on nursing care for patients in the last days of life, emotional support to families and advice on end of life care issues, and guidance with medications.

The team can also support the patient and their family at times of crisis to prevent unwanted and unnecessary hospital admissions.

The aim of Hospice at Home is to ensure that patient wishes, choices, and priorities are fulfilled where possible.
