Patients Groups & activities Participation & Respite Days “ …wonderful coming here…” “Thank you for the time your spent talking and helping” “ …always came home bright eyed and bushy tailed…” “Friendly welcoming place with kindness, care & compassion” If you have a life-limiting illness that requires regular monitoring you may benefit from our participation and respite days. Participation & Respite provides a relaxed and friendly atmosphere that allows us to monitor any changes in your condition and provide you with useful information including emotional support to discuss any fears or concerns you might be experiencing. The team comprises of a registered nurse, associate practitioner, health care assistants, volunteers and is also supported by a clinical nurse specialist. The day can provide a break for carers whilst longer term arrangements are made to support ongoing respite needs. There is also the opportunity to take part in meaningful activities tailored to your individual needs and preferences. All refreshments will be provided including lunch. If you wish to take part please speak with your key contact who can make a referral on your behalf. You will then be invited for an assessment to ascertain how we can best meet your needs and set your own goals for attendance.
