Hospice at Home

The Hospice at Home (H@H) service, works in collaboration with your District Nurses and GPs to provide a flexible nursing care service that supports your choices about place of care.

Who can we help?
We care for individuals who are in the advanced stages of a life limiting illness

What do we do?
● We can provide you and your family with support and care at times of crisis to prevent unwanted and unnecessary admissions to hospital, particularly in the last few days of your life
● We can provide additional support and care to assist in your discharge from hospice or hospital whilst other services e.g. District Nurse or social care are organised. Generally this will be 1-7 days at any one time
● We can offer planned supportive care visits to provide some respite to enable your family and those who care for you to continue caring, or to enable you to be supported in your own home if living alone

The support and care includes:
• nursing care
• help with washing, feeding and drinking
• listening
• being there
