



Farleigh Service Users are the people currently using Farleigh Hospice services who wish to be involved with feedback on current services provided. They may also be interested in being part of the decision making processes of Farleigh Hospice's future development.
Any of the following can be involved:
• Patients
• Carers / Relatives
• Supporters
• Bereaved Individuals
Service User Involvement
At Farleigh Hospice we are keen to enable service users to be involved in feedback on current services and also to be part of the decision-making process regarding changes at the Hospice.
Ways of being Involved
• Via completing surveys or questionnaires sent
• Giving feedback by telephone or email
• Meeting with the Independent service user facilitator
• Attending consultation forums
Forums
Where appropriate, we invite people to attend one-off forums for discussions on present or planned services. The forums enable staff at Farleigh Hospice to consult directly with patients and carers. By including service user views we are able to develop the services and policies ensuring they continue to focus on your needs.
